The Patient Partner Program is a full 12-week customised program designed to improve overall patient journey and outcomes following hip or knee joint replacement surgery. Supported by most private health insurers, the Patient Partner Program has been successfully delivering rehabilitation for total joint replacements at no additional out of pocket cost for the patient.
The 360 Patient Partner Program is delivered via a hybrid of face-to-face meetings and regular telemedicine video consultations to ensure you are well supported before, during and after surgery. Dr Roe and Jacqueline Chang work closely together to individualise each program to deliver your specific education and exercise activities, ensuring you receive the right information at the right time.
Prescribed by Dr Roe, eligible patients under the 360 Patient Partner Program will have access to a 360 Patient Partner Technology Pack:

Jacqui graduated as a physiotherapist from the University of Sydney in 2008 and has a background in musculoskeletal rehabilitation and rheumatology. Jacqui is a 360 Patient Partner who has been working with Dr Justin Roe since 2019 providing end-to-end care for patients undergoing knee joint replacement surgery.
When preparing a patient for surgery or supporting them through conservative management of their knee osteoarthritis, Jacqui is passionate about addressing a patient's lifestyle and knowledge, in addition to looking at exercise and dietary changes.
Jacqui works directly with Dr Roe to ensure their patients receive the best outcomes through personalised care delivered via a technology enabled rehabilitation program.
