Patient Partner Program
The Patient Partner Program is a full 12-week customised program designed to improve overall patient journey and outcomes following hip or knee joint replacement surgery. Supported by most private health insurers, the Patient Partner Program has been successfully delivering rehabilitation for total joint replacements at no additional out of pocket cost for the patient.
The 360 Patient Partner Program is delivered via a hybrid of face-to-face meetings and regular telemedicine video consultations to ensure you are well supported before, during and after surgery. Dr Roe and 360 Med Care work closely together to individualise each program to deliver your specific education and exercise activities, ensuring you receive the right information at the right time.
Prescribed by Dr Roe, eligible patients under the 360 Patient Partner Program will have access to a 360 Patient Partner Technology Pack:
- 3G enabled iPad (loan): access to rehabilitation exercise, online 24/7 messaging, telemedicine
- Garmin Watch: track physical step goal
- Cold compression pack
Patients receive this Technology Pack as part of their rehabilitation journey.
